Below you can access, e-sign and print out for your records the two consent forms we legally required before beginning any tattoo work. 

BOTH forms must be signed before beginning any work, and you must bring your photo ID. 

You may choose to fill out these forms in person at your appointment. However, e-signing them will save a lot of time when you arrive, which means Neri can get right to work on your new tattoo.
